You use the starting bullshit of whatever type you go with.  Sword, Hammer, Spear, etc.  Use whatever starter shit your god gives.  Don't use WWs.  Just follow the same rules as PAIN+.  Auto-leveling?  Probably want to reset each time you are about to win (unless forced to clear to unlock some trial).
